Enter the username and password provided to you by the Service Alberta Contact Centre to access the online consumer complaint forms and submit your concern to Service Alberta's Consumer Investigation Unit.

Once signed in, you will be prompted to choose the topic representing your complaint and complete a series of required and detailed questions about your experience. Space will be provided to add a statement and to upload scanned copies, images, or audio to support your complaint submission.

Note that while we continue development of this system, some topics require that the form be downloaded in pdf version, completed, and emailed with supporting evidence. Instructions are provided on the respective forms.
